General Information
The 57th Annual World Congress of the International College of Angiology will be held in conjunction with the 7th National Symposium on Vascular Medicine of the Indonesian Society of Vascular Medicine. This year's theme will be "Challenges and Opportunities of Vascular Medicine in an Era of Global Medical Services"

THE RITZ-CARLTON JAKARTA, MEGA KUNINGAN, in Jakarta, Indonesia, is the venue and headquarters hotel of the International College of Angiology 57th Annual World Congress and the 7th National Symposium on Vascular Medicine.

We encourage all attendees to arrive in Jakarta on Thursday, 1 October 2015. The Opening Ceremony of the Scientific Program will be at 08.30 h. on 2 October 2015. The scientific program will run from Friday, 2 October 2015 through the end of the day 17.00 h. on Sunday, 4 October 2015.
Language
The official language of the Congress is English.
VISA
All visitors to Indonesia are required to obtain a travel visa, either prior to arrival or upon arrival in Indonesia. Holders of a regular passport who are traveling for tourism, business or social purposes may apply for a 30-day visitor Visa-on-Arrival at airports in Jakarta. A VISA ENTITLES THE HOLDER TO TRAVEL TO INDONESIA, BUT IT DOES NOT GUARANTEE ENTRY. AN IMMIGRATION OFFICER AT THE PORT OF ENTRY DETERMINES THE VISA HOLDER’S ELIGIBILITY BEFORE ENTERING INDONESIA. Visa On Arrival
In accordance with the Regulation of Minister of Law and Human Right of the Republic of Indonesia Number 26, 2013, citizen from 61 countries and 1 region are eligible for obtaining Visa on Arrival (VOA).
This visa can be obtained directly when you are landed at the certain airports and seaport in Indonesia regardless of the purpose of your visits (Business, Tourist, Social). The Visa on Arrival is not a work visa nor a visitation visa. Therefore, it can not be converted to obtain other immigration permits. The maximum stays permitted for the visa on arrival is 30 days. If you plan to stay longer than 30 days, you need to mention your intention to stay longer. Visa on Arrival can be extended for another 30 days.
If you are entering Indonesia, you must have two fully blank passport pages in your passport. Indonesian immigration inspectors do not consider amendment pages in your passport to be blank pages. If your passport visa pages are nearly full, consider applying for a new passport or adding extra visa pages to your passport before you travel. If you don't meet Indonesian entry requirements, you may be denied entry with no recourse and put on the next available flight departing Indonesia.
The general requirements for Visa On Arrival are:
Expiration date of the applicant’s passport must be at least 6 (six) months at the date of entry.
At least one blank visa page
Round-trip airplane ticket
Visa on Arrival fee (refer below)
The fees for Visa On Arrival are:
Visa for stay up to 7 (seven) days in several Special Economic Zones (SEZ)= US$ 15,-
Visa for stay up to 30 (thirty) days = US$ 35,-
Extension of stay for up to 30 (thirty) days = US$ 35,-

Departure Tax
All airline passengers, including children, diplomats, and other public officials, are subject to a departure tax, which must be paid in Rupiah, cash only. The international departure tax as of October 2014 is 150,000 IDR (12.48 USD). The domestic departure tax in Jakarta is 40,000 IDR (3.32 USD) and varies elsewhere.
Registration Payment
Payment for registration fees may be made directly via our website: intlcollegeofangiology.org, by credit card, i.e. VISA, MasterCard, American Express, by check payable to the International College of Angiology in US funds and drawn through a US bank, or by bank transfer. For further information regarding bank transfer, please contact the College's Executive Director. Please note that Registration Fees are not transferable.
Cancellation Policy
A written request must be received no later than 15 August 2015, less a (USD) $50 administrative fee. There will be a 50% processing fee for all cancellations received after 15 August 2015 and before 15 September 2015. Fees are non-refundable after 15 September 2015.
Insurance
The Congress Secretariat and Organizers cannot accept liability for personal accidents or loss of or damage to private property of participants, students and accompanying persons, either during or indirectly rising from the International College of Angiology 57th Annual World Congress and 7th International Symposium on Vascular Medicine. Participants should make their own arrangements with respect to health and travel insurance.
ABSTRACTS
Participants interested in presenting an oral, video or poster presentation are invited to submit their abstract directly via the congress website for consideration by the Scientific Committee. Please see Abstract Guidelines page and Abstract Form for online submission. In the event of difficulties in submitting your abstract through our website, please contact the ICA's Executive Director: denisemrossignol@cs.com or denise.m.rossignol@intlcollegeofangiology.org Please do not submit multiple copies of the same abstract. If submitting an amended abstract, please indicate that it is a replacement abstract.
Symbols, tables, graphs or pictures cannot be submitted via the website. Abstracts containing symbols, tables, graphs or pictures must be e-mailed as a Word document attachment to the ICA Executive Office: ica@intlcollegeofangiology.org and/or the Program Chairman, John B.
